(I wonder if the lack of references and index is a product of self-publishing? Did you read/does it draw on Happy City. Another excellent book on similar turf).

The original version published in the US by Abrams does have an index, but annoyingly we never found a UK publisher and so in the end I just self-published. I feel bad about the index - I have a plan to create a post probably here with extensive notes. Hopefully dedicated readers will find it! I'm so glad you enjoyed it.
